In the process now...
In case you get the message that no record of your phone was found, make sure that you're accepting cookies and have scripting turned on. I have Firefox set to ask me for cookies every time, and http referers turned off and kept getting the error.
I switched to Safari, and all was well.

Follow up: the process works.
(1) Enter your iPhone phone # and serial number.
(2) Wait for the free SMS text message that will have the access code. The web page says up to five minutes, but mine took 15 minutes.
(3) Enter the access code number on the web page as prompted.
(4) A $100 store credit is created and shown on the web page. PRINT THIS OUT! It has the credit # and PIN that you will need to use.

Mine took over 30 minutes. Had to do it twice, because after not receiving any SMS for over 20 minutes the first time, I got worry. Second time I did it, I got the SMS within 5 minutes.
