I know that this problem has been seen before but this is a new observation.
1. I purchase a song from my iPhone and try to add it to a pre existing playlist all from my iPhone.
2. I then go sync the iPhone to iTunes on the computer, the added song on the playlist does not get added afterwards.
3. Same thing applies to redownloading songs from the cloud.
4. However, I am able to use my iPhone to add songs that are already on my phone from my iTunes library via syncing to a playlist and the changes will be retained after syncing.
5. If my wife purchases a song from iTunes and we both have automatic download enabled, the song that gets transferred to my iPhone IS ABLE to be added to playlists and the edits remain after syncing. However, my wife is not able to add the exact same song to her playlists and have it be retained after syncing.
This is the work around:
1. if you purchase a song on the iPhone, you first have to sync with iTunes on the computer. Then transfer the purchases to the iTunes library.
2. Then you have to delete the copy on your iPhone.
3. Then you have to manually add the song from the iTunes library back to your iPhone .
4. Then that song will be able to be added to any playlist on your iPhone and have the edited playlist retain the changes.
This needs to be fixed. Ideally, one should be able to add/edit playlist songs and have all the changes be retained after syncing. I have heard of people having huge library of songs that are in playlists be removed after syncing.

i have iPhone 4s with iTunes 10.5 using mac os lion.
