It is a Motorola StarTAC ST7868 connected to a Palm m100 via a cable I bought from
www.thesupplynet.com. (This phone also has WAP support.)
My service provider is Verizon Wireless, whose cellular service has a built-in ISP at no extra charge. Data calls subtract from your minutes just like other calls, but other than that there are no additional costs or limitations on data use. With the Mobile Internet Kit, you can use PQA's, and you won't be charged per kilobyte like with Palm.net.
Cables from TheSupplyNet usually come with really good documentation about how to set up a Palm for use with Verizon or Sprint PCS, and others. For other providers, you may need to use your own existing dial-up ISP.
