So i have a USB and used it in my MacBook everything is fine, then today in school i placed the USB in a Dell computer which runs vista. It says that the format is wrong (it was a E drive instead of the usual F) and asks if i wanted to reformat the USB and i said NO.

Then today when i got home i placed the USB inside my MacBook and it says that the disk cannot be read!

Please help me, what should i do? Anyone way to make the USB work for BOTH vista, mac & XP? This is really bugging me since other USBs don't really have this kind of problem. Thanks!

USB Type: Transcend JF V30 / 4 GB.

Sounds like you yanked the drive out without unmounting it (on either OS X or Windows) and corrupted it.
Reformat the flash drive as FAT32 and restore your data from backups.

In windows i clicked remove disk while in the Mac is ejected the disk into the bin. Isn't that how you do it? Unless i'm not doing the right thing.

Same thing happened again. I reformatted it on a Vista, then i did it again on the Mac. Stored some files and checked if it was okay, it was... then tried it on a Vista and it was also okay.

Then i went to school placed the USB in a XP computer and it said cannot open disk/disk not there so i ejected the disk. When i got home, i placed the USB into the Mac and it says the disk can't be read...

Possible that the problem is in the USB?

You're calling it a "USB". Let's instead call it a USB drive.

There may be a problem with your USB drive. They're cheap, I'd replace it.

Ok thanks. I'll try to reformat the USB drive once more.
