error formatting/erasing card

When I insert a mini sdhc card into the usb port the mac states that it disk is not readable by this computer and either initalize, ignore or eject.

I try to intialize and it will show up in disk utilities. I try and erase it and get disk erase failed imput/output error. My palm phone reads the card the card is new the adapter says it is compatable with mac the mac sees the card (memory amount ) but it won't erase or let me try and repair. I did not see an option to format but not sure if that is my problem. Please help....

is there a lock on it? have you tried formatting it in your palm treo?

Could not format witha windows pc and the mac won't see it. The card works with the treo though. Just can't get it to read....on the computer
