How can i make a quicktime movie that has only 1 frame saying 'click to play' and it load the rest when it's clicked?

you need to create a poster movie and have it link to you real one .. more details at;

What happens if the person coming to my site does not have QT at all? What will show up in the web broswer, broken plug-in images?

Pretty much. You can write some javascript to detect quicktime at all and/or write some quicktime refferences to detect what version of QuickTime they have.
