I registered for QuickTime 4 Pro and I just noticed that it doesn't work on QT5. Do I need to purchase this again or do I just need to get a different registration number as a registered user?
If you purchased a QuickTime Pro license before October, 2000 (or whenever QT Live! was), then you need a new license for $30. All licenses purchased after the magical date will continue to work.

It was for a while. A license for 3 worked all the way through 4. I guess 5 has enough added enhancements that they felt it was necessary to make a new License for it.
