G4 shuts down at random. Why?

Have a friend with a year-and-a-half-old Quicksilver G4 (867 MHz), running OS 10.2.6. A month ago, he reported that his CRT monitor was sometimes not picking up the video signal at startup. Sometimes restarting would restore the picture, sometimes he'd restart many times before it would return. Eventually, he stopped fighting it and bought an Apple 20" Display and was very happy....until....

A couple weeks ago, he reported that his G4 was being flaky. Would turn itself on in the middle of night. Would shut down randomly while being used. And about once every five reboots, it would startup into the Open Firmware prompt. When he typed "mac-boot", it always boots into OS 9.

Energy Saver settings seemed normal. Noticed that when we adjusted the Energy Saver settings, the Preference, "com.apple.finder.plist" would show an updated Modify Date, so we trashed that. (Is there another pref we should trash?) Also zapped PRAM, and tapped the "CUDA" button on the motherboard, but no change.

So, thinking it had to be some sort of power spike issue, I had him go out and buy a UPS. Didn't help. Then I had him buy a new power supply. Installed it. Still behaves exactly the same.

Any ideas what's going on?

Might sound silly... but has he tried a clean system install?
