If your PB is running, close the lid to put it to sleep, then plug in an external monitor and keyboard. It should wake automatically in clamshell mode; if not, hit a key to wake it. If the PB is off, then plug in the monitor and keyboard and (depending on your PB model) start up from either the external keyboard or power button, and quickly close the lid.

Yes, basically you have two other options. The PowerBooks and iBooks knows when to go to sleep because of a small magnet under the right arrow-key (at least it's located there on the iBooks) that triggers something in the lid when closed. If you screen this magnet off with a coin, for example, the computer will not fall asleep.
Another option (in my opinion much nicer but a bit more complicated) is to patch your kernel with a no-sleep-patch and recompile it. Instructions can be found at: http://www.binaervarianz.de/projekte...ieren/meltmac/. I've done this with my iBook and it works great.
