It refers to the Mac OS X wait cursor which is a colorful spinning disk. It's also known as the "beachball cursor" among other names.

The "of death" is sometimes added to the name when the cursor remains spinning for a long time, usually as a result of a bug or other problem. The usual cure is to "force quit" the application which produced the offending cursor (press Cmd-Opt-Escape to do so).

Wow, glad I read that! I used to start Activity Monitor to force quit an app. Thanks!

Another force-quit method:

Most beach-balled apps will automatically change the dock menu's "Quit" to "Force Quit"

If it still says "Quit" in the dock menu, hold down option and it will change to "Force Quit"
"You rise," he said, "like Aurora."

Yes, that's actually the technique I use most often. Thanks for pointing it out.

you can also quit things via the activity monitor (applications/utilities). One called 'BomArchiver' or similar has a tendency to lockup on me and doesn't show in the dock, so this is how I quit it.
