I'm looking into buying a Macbook Pro for personal use as well as work, and this is what I've boiled it down to. I play some games and would like this to be good for medium-intensity gaming. However, I don't want to spare some money only to regret not updating later. Something else that concerned me was the Apple Care protection... Specifically, whether or not it was worth it. I'm also debating about whether or not I should wait for Mac OS X 10.5 to be released.

If you really need that few Ghz more than go for it, but otherwise, it does seem like quite a bit $$ for only that difference. I would go for the 2.16 model.
Good choice of the size, whichever model you choose, I think the size of the 15" is great!

First option looks good.
The .17 Mhz per core isn't worth the money.

I personally think the extra speed, RAM, and video memory is worth the bump to the 2.33 model if you'll be using it for a long time.

What about waiting for Mac OS X 10.5? I don't want to wait much longer than 2 months tops, but some are saying that it might not come out until late summer.

You said that you play some games? The extra 128MB on the graphics card would go a long way in providing you better performance for those high end games.

You mention playing games. So the extra MHz and VRAM will be of advantage.
Both extras cost a bit, but keep in mind that these extras are soldered to the board - if you don't get them now, you never will. If you're planning on keeping this one for more than a year and you play games, get the 2.33 GHz model.
