If trashing the preferences does not work, the easiest way to reinstall it is to pop in your OS cd and reinstall the OS. Not a clean install, just reinstal right over the existing OS. You don't even need to backup your extensions or anything, they are all left alone.

Or download Open Transport and just reinstall that. No CDs, no risk of anything else getting messed with (even though they shouldnt be.)
If you have another mac, just copy OT extensions from that and replace.
