I noticed this in UT04, BUT NOT in UT03, (or I beleive the demo, i'd have to reinstall it to check...) I have assigned the Command key to be "crouch". When I fire from the crouched position, Command-Left Mouse button , it ends up being the "alternative fire" mode for weapons, as opposed to continued crouch plus primary fire.

I had reported this as a Bug, the response from Ryan (the programmer) was a simple fix:

Since most mac users have a one-button mouse, we have cmd-leftmouse act like the
second mouse button.

Look under your Home directory for Library/Application Support/Unreal Tournament
2004/System/UT2004.ini...it's a text file.

Look in that file for "MacFakeMouseButtons=True" ... make it
"MacFakeMouseButtons=False", and make sure you save the file as plain text and
not Rich Text Format. Your problem should go away, then.

Hope this helps....it sure makes a difference for me...

I did post back that this may be a good option to enable in input preferences, as most users probably do have multibutton mice/trackballs that are into gaming, and most of us (ok, at least me, but I'm new) wouldn't have known to go in and edit this thing...

good luck

Many thanks! I prefer left-shift as alt-fire anyway, when using trackpad or monomouse. And I like Cmd for Crouch. (The demo didn't have this issue--now I know why.)
