Myth TFL quits immediately after the intro movie to the first map (Crow's Bridge). The first map with the scrolling voice / print introduction plays fine, an intro "movie" then starts (two guys on a bridge, one eating an apple). Immediately after this movie ends Myth quits back to my finder. I can't get past this spot. If I hit escape to quit the movie Myth quits the same way. I've upgraded to version 1.3, virtual memory off, increased the memory of Myth to 64 megs, decreased the colors to thousands (all per the FAQ page) and cleaned the CD (no spots noted on the CD), and have a ton of free hard drive space (1.5 gigs free on the games partition).
G4 AGP model with RAGE 128 16 MB of VRAM, OS 9.0.4, all extensions and control panels up to date, 320 megs of real RAM.
This copy of Myth is an older version (if that matters). I purchased it several years ago but didn't have a Mac at the time that could run it. I only recently was able to upgrade to the G4 and have begun playing the games I've accumulated over the last couple of years in anticipation of getting a Mac that could run them. I also have Myth II but haven't tried that one (guess I should have waited and just bought the Total Codex but I didn't know it would be released and didn't want to miss owning Myth). Anyway, any ideas why it quits on me?

I have a setup very similiar to yours. I'll install Myth: TFL and tinker with extension sets and such to see if I can get it to work. I can get in touch with you via e-mail if necessary.

I installed, updated to 1.3 and it ran perfectly in Software on my built-in Rage 128 Pro and in Software and 3dfx on the Voodoo5.
I think the problem is in your Rage 128, or more specifically, in the current Mac drivers from ATI for the Rage 128. The Rage 128 has some proven problems that the Rage 128 Pro doesn't exhibit. I'm guessing that these kinds of problems will rear their ugly heads on an occasional basis with some games until ATI addresses the situation.
If you absolutely want to play Myth: TFL, then I'd recommend picking up a V2, V3 or the pricier V5 (if you were in the market for a video card anyway). Unless you have multiple monitors to use, I'd pick up a Voodoo2 because then you can keep the DVD and Quicktime acceleration of the Rage 128 and also the better, more stable 3D acceleration of the 3dfx card. A Voodoo2 should run around $50 or less, the Voodoo3 $50-$100 and the Voodoo5 will cost around $300.

Bumping up the memory for Myth did the trick. I upped it to 128 megs and now it's running fine.
