Form troubleshooting woes

Hello!
I am having some trouble trying to figure out the form I have on this <a href="http://www.cloudninedayspa.com/info.html">page</a>. It's a pretty baisc form--I just want the results emailed to the info address. Everytime I test it, I get 404 not found. This has been plaguing me for months now and I really want to finish this! I set the permissions and everything. I'm afraid I might have messed up something in the script--referrers or arrays? I would be forever indebted for any assistance!

does http://www.cloudninedayspa.com/cgi-bin/formmail2.pl actually exist then? that's where your form is pointed, and sure enough, i got a 404 too. are you completely sure about your cgi-bin's path?
can you use php on that server? if so, just grab this script and drop that in your main directory, instead of the perl one

Wow! That one looks MUCH easier! I have two questions--I just pop that into my cloudninedayspa.com folder on the server and then point the form to /formmail.php, correct? Also, when entering the referrers, is the form hosted on warped.com (the webhost) or cloudninedayspa.com? That's probably a stupid question, but I just want to make sure.
