Best way to do local area Emailing for business?

Greetings,
I'm trying to help out a local chiropractor by trying to do an email campaign for the local area (los angeles) ......
.... are there such things as companies that can help target a local market for emailing? (through an email list maybe?)
Something that's targeted, and not just a mass email (I don't want to spam).
Any ideas? (companies in mind?)
Any help would be greatly appreciated!!!

Idea: Don't do an email campaign.
I can't think of a single way to do an 'email campaign', other than one specifically signed up for by the recipients, that won't be a) ignored and b) spam.

How about a more targetted online campaign, where you use advertising banners on sites that are more suited - ie. local councils, chiropractic associations, doctors clinics, community billboards, sports groups. etc...
It's less intrusive and potentially a better spend because you know you'd be reaching the right audience. You can also get slightly better click-through figures...

but regarding the email campaign : is that a bad idea as a whole? there's no such thing as email houses that can help target specific clientel?

Most companies who work with mass email marketing retain their lists while providing you with broadcasts for $$$. Their compiled email addresses are their main asset, and they aren't likely to simply hand over thousands of addresses for a fee.
I suggest going with some seach engine marketing. Go here and type in "chiropractor". You'll see that over 1300 searches were performed last month through Overture's network (owned by Yahoo, results span many search sites like Yahoo and MSN)) for "chiropractor los angeles".

Here's what I recommend:
(1) Make sure the chiropractor's website is cleanly designed with simple navigation.
(2) Set up an Overture account. You can set monthly budgeting. Target keywords "chiropractor los angeles". If you choose additional keyword phrases, make sure that they are not too general. For example, targeting just "chiropractor" would mean you'd be paying for hits of which the majority are not in your desired audience.
(3) Develop a promotion w/ the chiropractor to help reel in these targeted hits. Something like a free exam and X-ray... a $250 value (and mention that it's a limited time offer). Have the Overture campaign lead the hits to this special web promo page. And instruct the user to "call today and mention promo code XXXX" and/or see if they can fill out a brief contact form.
Ultimately, especially for chiropractor-type businesses, it's best to have other marketing campaigns working at the same time. Direct mail is nice, and would work much much much better than a mass email campaign (no envelope though.... an oversized postcard-type thing with the "special offer" would be fine here, ala Bed, Bath, & Beyond, or Linens and Things. Requiring targeted prospects to open up an envelope drastically reduces promotion visability and effectiveness.
I've specialized in this stuff for many years, and my brother is a chiropractor, so I feel as if I have a good grip on your specific situation. The web doesn't generate nearly as much busness for him as more traditional methods (direct mail, newspaper ads). But he's in a small town in South Carolina. After looking at those Overture numbers for people searching for "chiropractor los angeles", you have a much better chance at generating web leads than he does.
One more thing... make sure you check out the competition's web site(s). Users will often check out a few of the top or sponsored results. If their sites are nice while your site looks like it was made by someone's 12-year-old cousin w/ Microsoft Front Page, you may not stand a chance at reeling in the user.
Yahoo search for "chiropractor los angeles"
MSN search for "chiropractor los angeles"
NOTE: Google is the most used search engine, and you may want to look into their Ad Words program. I focused on Overture above because in my recent projects, Overture has provided the best cost-per-lead ratio. That may or may not be the case for you. It takes a little experimenting and monitoring to get an optimal balance.

In regards to local postcard-style mailings ..... are there mailing houses [or something like that?] that will provide addresses that will more or less, target a specific demographic/injury-type within that local area?

I'm not sure. Instincts are telling me no for "injury type". That's medical information, and we have a bunch of laws on the books regarding the disclosure of such information.
What you can get are lists based on lifestyle data, like those into phyical fitness/excercise. You can also target certain ages/genders... all within a specific geographical area.
Try the "free list count" feature at http://mip.usadata.com/usapub/ .... you'll at least get an idea of the different demographical breakdowns and costs.
Also, I suggest Googling for "Mailing List Brokers". While many may not be actual mailing houses, they do have the more valuable commodity - the lists. And many can point you to some (mailing houses tend to also be print shops) if you are unable to locate one yourself.
Don't hesitate to Google whatever ideas or questions you have. You'll get answers a lot faster than waiting for me to respond here. Besides, if I tell you any more, I'll either have to charge you for tuition or consultation.
