Help With Simple Forms Processing and FormMail.pl

I am trying to learn how to process forms using FormMail.pl by using this demo/tutorial code. I put it on my host's server (which has FormMail.pl on it), and I changed the email address it mails to to one of my addresses. I submit the form, it shows the submitted result, but I never receive the email. Any ideas?

I called my host's support line, and while they did not know why the script was failing they pointed me to the cgimail and formmail tools in the host's control panel. I still had to solve the problem, and it finally occurred to me: any script that directs mail to a local mailbox fails, presumably to prevent bounce loops. I think that was the problem. Now my form works.
