Click the "Change all..." button when you're changing it for one file of that type?

How are you changing the default application for one file?

The "change all" button is right there when you get info on the file in Finder (Apple-I) and click open the "Open with..." triangle to select the application to be used...

You can also hold down the option key while selecting an item from the "Open With" submenu of the contextual menu ("Open With" changes to "Always Open With").
