I tried searching but didn't find any info on this here. I did find some leads using Google.
Is there an inexpensive Mac program that allows me to store and track client/patient info and notes? (I don't care about billing or invoice)
The psychotherapy/counseling ones like TheraManager $495, ShrinkRapt $585, and TherapySOFT $385 are too much for me.

I would seriously suggest "Omni Outliner Pro". go to the link below:

If taking notes and organising them by date is all you are interested in then you can do alot with this application. I find that 'purpose built' applications can be a pain to use because they lock you into doing t hings 'their way' and they also have features that you may not end up using. So you maynot like to do it their way and would prefer to do it another way.
I reccomend OmniOutliner it because it is extremely flexible and simple and you can make it represent anything you want how you want. and it is cheap!  (It comes with the mac osx anyway so you might already have a copy!!!)
I currently use it for structuring algorithms for my software development and also for 'todo' lists and i am even writing a fiction novel (trying lol.) and this program makes it easy to organise all my info and thoughts.
Take a look at it for yourself though... it has much information and plenty of screen shots.

I'm looking for the Mac equivalent of TheraScribe 4.0 ($175), a psychotherapy treatment planning system.
[UPDATE:] I emailed them and they said that they're not planning to do a Mac version.

This is an 'open source'(what it says on apples products guide) medical practice thigy that seems to be along the lines of what you are looking for. open source is usually free...
Hope this product helps ya... but if it doesnt and you want a place to search for specific mac things, you can look further through the catalogue at :

and the medical DMR kind of stuff is under the section of "Business & Vertical Markets"

Methinks this would be a ideal application of Core Data. Hell, it's even easy enough you could probably create your own custom app.

Unfortunately, I have zero coding skills. No technical knowledge that would allow me to create one. Just a good consumer of developed softwares. Man, I would buy the $175 TheraScribe. It's expensive (to me) but reasonable. But no way am I dishing out $400-$500.

from them screenshots, it looks like something that could be recreated in Filemaker.
have a look on their website http://www.filemaker.com/ and see if anyone already a solution/database that suits you. If there are missing a feature they* maybe able to add it to a existing module for an extra payment.
or search on google for a match Filemaker based solution.
*they being the database developer, not Filemaker

Nivag: Yeah, FileMaker looks like it would do some or all of the things I mentioned. I've never used FileMaker before but I might just download and try it out. Still, the search continues.

it's a powerful bit of software, i've used it on and off for a number of years from simple contacts to working for a import/export company running 13 different bank accounts and a shed load of products, it all worked very well.
after awhile the import/export company migrated to a custom built 4D based database - this was because they was running 3 different firms from the same warehouse.
they director who wrote the database spent many a sleepless night getting it sorted. it works a treat!

Like I said earlier, the general data storage/entry would be pretty trivial in Core Data, but there's a lot of other functionality TheraScribe provides which would be more difficult to implement.

PsychReport is $200, though I'm guessing you already found it on Google and didn't like it.

Yeah, thanks though. I did test out PsychReport (briefly) but found it "clunky" and its user interface outdated. I'm holding out for the Mac version of TheraScribe. Well, if I get a new MacBook Pro, I could use TheraScribe through BootCamp.
