I have set several birthday reminders etc in iCal and have set up iCal to provide me with an email when I am within a week of these dates. This is suppose to work even if iCal is not up and running. However, it doesn't seem to work at all unless I start up iCal. Has anybody out had any experience with this one?

Have you checked in Preferences > Advanced > Turn off alarms when iCal is not open.
