Maddeningly, Adobe Acrobat Professional 7.0 includes Adobe Designer formerly Adobe LifeCycle Designer) with only the Windows version; the Mac version apparently has no ability to create Fill-In Forms with calculations, which is what I desperately need.

Is there some other product that can do this?

You can create fill-in forms with Adobe Professional, it is a horrible interface, and you have to do formulas manually, but it can be done. I have been petitioning the OmniGroup to do this in OmniGraffle.

It's actually pretty rudimentary. You use a half-ass version of javascript. If you do it on windows it comes with an extra application that is absolutely beautiful for designing forms called "Form Designer" it does most of the work for you.

Download a free 30 day trial of Adobe Acrobat Professional 7, and you can easily create forms. Calculations are a bit harder.

First, I wouldn't call the javascript (ecmascript) support in Acrobat 'half-assed'.
Anyway, I wrote a tutorial on creating business forms according to international (ISO) business forms standards. There a link to an indesign template, should you use indesign. Regarding the fillable fields, those are created in Acrobat Professional. The interface takes a while getting used to.
