Software to create printable forms?

Hi everyone. I was wondering if you all knew of any software that can create custom printable forms (such as a personal information form that you might fill out at your doctors office). I am kind of new to this so any info would be appreciated. Thanks!

I'm guessing Illustrator or Quark/InDesign.

Forms aren't fun to design...

Apple's own iWork may do the trick, without spending a fortune on InDesign, it is only $79
