I'm looking for an app to plan web site development - like database schemas, modules, notes, how things work, how things relate, etc.

Any recommidations? Voodoo Pad and Omnioutliner does not do the trick.

I know I've done some site mapping work in OmniGraffle and that's worked well for my needs. Sounds like you need something a bit more complex tho'...

I like Omnigraffle, but I need something that combines the features of Omnigraffle, Omnioutliner, Word, Voodoo Pad, Yojimbo, etc. I need chapters and anchors that I can link to from other portions of the "document" and I need to be able to attach notes and comments.

You can (possibly) do that in OmniGraffle Professional alone. Notes can be appended to objects and you can use the Actions palette to link to internal objects, URLs or external files.
