I know I'm a few years late to the party (bergy originally referred to this app in 2004), but I just found PDF Shrink, and this is high quality, polished shareware that would have saved me a tremendous amount of time over the years. I highly recommend it to anyone who processes PDFs regularly.

This app plus Monkeybread Software - Freeware - Combine PDFs 2.1 are essential downloads for PDF authors.
