is it possible to manipulate PDFs to add an image?

I know it's possible to use Acrobat Professional to edit/manipulate certain aspects of PDF's; e.g. in addition to highlighting or annotating, one can sometimes add or delete text. I'm wondering though if it's at all possible to add a graphic into a PDF? Can Acrobat or any other PDF-manipulating program do this?

You can convert it into another format, and the image you want, and then convert it back to PDF. Acrobat may have more options.

Only thing I've used successfully is Illustrator.

What sort of PDF is it? Was it generated using Distiller or exported using a plugin or through the print dialog's "Save as PDF" option? Lots of image apps will open and edit PDF, but they edit it as an image. Not much in the way of object manipulation.
