what am i forgetting? (trouble with color bands in photoshop)

I can't remember what I'm doing wrong, but I keep getting horrible bands in photoshop when creating gradients. It looks like my monitor is set to thousands of colors, but it isn't. If needed, i can post a screen shot, but I have a feeling there's something small that I 've fogotten.

I was trying to make a gradient that fades from black to white to use to modify the skyline of an image, but I have the same trouble with any set of colors.

Also, the same thing is happening with blurs, or anything with blends between colors that I have created.

when going to press it helps to add a little noise to soften the transitions. if that's not what you're looking for please post a link or three.
