I understand there's a number of factors to be taken into regard (like color profiles, for instance), but is there anyway to convert the Pantone color codes to hex color codes (like #E6B34D) for applications like Photoshop?

I don't know if it's any help, but I have a conversion chart for Pantone to ink color formulas for printing. There are three columns with headings as follows: PANTONE Color, PANTONE Process Color, PANTONE Hexachrome Color. The entries look like this:
PANTON 1365 | C-0 M-29 Y-72 K-0 | K-0 C-0 G-0 M-0 Y-41 O-33

Yeah... when you are in the Pantone Swatch Dialog pick your colour, hit the "picker" button and it'll bring you back to the more traditional photoshop colour pallete.. on the right you'll see all the colour #'s for CMYK, RGB, Web.. etc... or it should.
