Illustrator keeps converting CMYK to RGB!!!

Help guys! I am working on this document in Illustrator in CMYK mode. For some reason, after I save it and then open it again, it opens in RGB mode and converts all the colors as well, i.e. if I was using C0 M100 Y100 K25, it will convert it to like C20 M97.65 Y92.55 K6.27. WTF! Why does it keep doing this? I convert the colors back and then resave in CMYK but it keeps converting to RGB mode  help ASAP please! I have a deadline tomorrow.

i think i know what's causing the problem but i don't know how to solve it. i am working with an .eps image made from photoshop. i think it's the way that i saved it that is causing to convert the document to RGB. ok, when i am saving an .eps image from photoshop, i go into save as menu and then at the bottom there are 2 checkboxes for Color:
1. Use proof setup working CMYK
2. ICC Profile US webcoated (SWOP) v2
Which of these do I check/uncheck to work in CMYK mode? I think I had the ICC profile checked. Is that the problem?

GODDDDD. what is up w/my Illustrator today. i tried just creating a test CMYK mode document. i put in a bunch of cmyk shapes/colors and save it. i open it again and it is converted to RGB!!! wtf!! what is going on?? this hasn't happened with the past cmyk documents i've worked on before.

I'm trying but I can't replicate the problem. I know the same thing has happened to me. I recall something about switching plat forms or a plced image.
I'll see if I can help.

nope- your pshop setting are fine.... are you changing versions or platforms? Are you saving as AI? What version of Illy?

Hi iChelle
If you haven't already:
1. Be sure you have selected CMYK Color ( File>Document Color Mode>CMYK Color ).
2. Change your swatch window to "List View" which will show colors listed with RGB, CMYK or spot colors (Pantone) Icons.
Any RGB color used can then be ID'd. and can be changed by individually double clicking on the swatch and changing the color mode.
3. Select all, then convert colors ( Filter>Color>Convert to CMYK ).
4. Save the doc.

thank you for all the advice. i followed each and every one and even did a reinstall but it is still converting every CMYK document i make into RGB. I have checked that the document color mode is CMYK and that all the color palettes are as well. I do a save and after it saves, it is still CMYK. HOWEVER, AFTER i exit out and reopen the document, it gets converted to RGB colors.  i have this huge deadline...

i know what happened. i just did a reinstall and everything worked fine again. but when i tried to place an image, it told me that Pantone #xxxxc is missing. Would you like to add or merge the swatch. i clicked merge. now it is converting everything to RGB again. how do i undo that swatch merge?
The prompt that was screwing Illustrator up was this:
The color swatch Pantone 2945CVC has a conflict with the current document.
Options:
Merge
Add
(OK button)
Selecting merge and OK was what caused the problem. However, I am trying to place the same image again and it's giving me that prompt. Should I just select "Add"? (Illustrator 9)

If you can, correct the original image, changing the PMS to CMYK.

In Photoshop: Yes you can. You can add, delete and change individual channels. The final file must be multichannel and saved as a DCS EPS.
In Illustrator: Yes you can.
