I use Fireworks, it has a set of effects that do that automatically (beveled edges and drop shadows)

Doing it by hand is still the preferred way for a lot of people. Letting photoshop hold your hand doesn't allow you the amount of control that is needed most of the time. Those layer effects are good for small things, but don't let them be your crutch.

Yeah, you know in Fireworks, I generally just use Beveled vectors?
haha, but then again it's different than the way Photoshop is set up for.
I wish I had something more than Photoshop LE, sigh, I'm still waiting for Photoshop Elements to carbonize.

Basically, as it reads.
Create a new layer.
Hold control (in this case, the command button since the tut was used on a pc) and click on the pill shape layer.
The new layer will then be the marquee that was made using the second command in the second step.

Command + click any layer to load it's pixel contents as a selection.

The marquee tool...should be the first one in the top left corner of the tools palette in Photoshop. From there you can choose eliptical and rectangular marquees.....dashed lines to make a selection on an image.

AH!!!!
Finally figured out what they meant.
After you make the second layer go back to the background layer. Use the magic wand to select the border of the object. Then the Select->Modify->Contrast will shrink it in to get the inner ovel we want.
