In this thread, we'll point out and discuss major airplane crashes over the years.
No straying off topic either...
I'll get it started.
From Wiki:
Japan Airlines Flight 123 was a Japan Airlines domestic flight from Tokyo International Airport (Haneda) to Osaka International Airport (Itami) on August 12, 1985. The Boeing 747-146SR that made this route, registered JA8119, suffered mechanical failures 12 minutes into the flight and 32 minutes later crashed into two ridges of Mount Takamagahara in Ueno, Gunma Prefecture, 100 kilometers from Tokyo. The crash site was on Osutaka Ridge (御巣鷹の尾根 Osutaka-no-One?), near Mount Osutaka. All 15 crew members and 505 out of 509 passengers died, resulting in a total of 520 deaths and four survivors.

Swissair Flight 111. A regularly scheduled flight from New York to Geneva.
It was 1998, a bomby September 2nd day. Swissair 111 took off and was flying close to Halifax, Nova Scotia when the pilots smelled smoke in the cockpit. The cabin crew was alerted and they figured it was the air conditioning, so they turned it off. A few minutes later, the smell of smoke returned, and it was stronger, as well as visible. They then radioed into the Moncton Airport and declared a pan, pan, pan. That's code just short of an emergency.
They ended up being vectored into the Halifax International Airport. Well, the rest is history. They never made the airport. 229 dead.
I partly blame Bill Pickrell, the Air Traffic Controller in Moncton who, knowing it was a pan, pan, pan, still diverted the plane to dump fuel. The plane was only 56 km from the landing strip. His story is mixed. On the one hand he said the pan, pan, pan was treated as an emergency, then on the other, he said he didn't think it was much of an emergency and treated it as routine.
I agree with the Airplane Safety Analyst in the documentary. Where there's smoke, there's fire. Land the plane, now. I would have either just dumped the fuel and landed the plane, or just landed without dumping.
Bill Pickrell should not have diverted them to dump fuel. He even heard the transmission about the emergency checklist for broken air conditioner that wasn't intended to be broadcast from the cockpit. Plane needs to make an emergency landing. It's a pan, pan, pan. What more do you need to know, Bill?
What happened: advanced entertainment system in first class caused some kind of arc in the wiring/battery short that caused a fire inside the walls of the plane.
Blame game:
ATC: Bill Pickrell for diverting the plane to dump fuel, and not bringing them into land in Halifax. This caused the plane to be in the air longer.
DuPont/Swissair/FAA: for not having proper fire retardant materials standards.
Swissair: for not putting even an off switch for the advanced entertainment system in first class. There was no way to shut it off without cutting all electrical systems out.
Swissair: for not thoroughly and properly training pilots and including in their checklists things to cover safety with the new entertainment system.
