AFAIK, if you buy it online, you don't receive anything other than an e-mail to show you have purchased it. If you buy the "boxed" version in an Apple Store, I think you get a piece of paper with a registration number which you can input when registering your purchase online.
Once registered, you will be able to use iDisk, download the freebies on offer from the Apple .Mac web pages ( www.mac.com), use your .Mac e-mail account in your e-mail app and online, etc, etc.

I just renewed last evening with a boxed version. In the box was a CD with the software for .mac (iSync, etc.)that you can actually download from .mac. On the CD envelope was the serial number I used to renew. Hope this helps!

If you sign up online then you have already put in your username and password.
If you buy the retail package there is a software CD and instructions inside containing a code that you will enter when installing the software.
Either way pretty easy. Once set up make sure the apps are up to date and visit the .mac page to see what offers are there.
Note: if you set up the Update program to check every week for updates, you will be able to upgrade your OS X apps and system on a regular basis.
