is it possible to make a disc image that when mounted has an icon you created (instead of the default white HDD-looking thing)?

also, some disc images open up to show a folder that has a specific background and layout - you know the kind, they say stuff like "drag this icon to your apps folder!" using a nice image graphic. How do you do this?

I have a feeling the answer is simpler than I imagine...

Simply paste the icon onto the disk image when mounted. With the desired image copied, mount and select the image, Get Info (File > Get Info), select the icon, and finally paste (Edit > Paste).

The background image is done using an image that is on the disk, but hidden in some way. The background is set in the Finder (View > Show View Options, and set Background to Picture).
