"Ghost" Bluetooth device stuck in Audio In/Out list

I upgraded to 10.4.3 yesterday. I have a Plantronics headset in my Audio Input/Output list even though I deleted the device from the Bluetooth favorites. I don't know what I did wrong, or in what incorrect order.

Anyone have a solution for deleting this entry in my Audio prefs?
