keychain for safari? (jpg inside)

How can I remove this? Eveytime when I log in my account and clicked on Safari, this window pops up in front of Safari - asking for password..
I remember something about going to "Keychain app." and fix this error?? Help? Thanks

To set up automatic keychain access for an application:
Open Keychain Access, located in Applications/Utilities.
Click Show Keychains if the Keychains list is not open, then select a keychain.
Select Application in the Passwords category. (Click the disclosure triangle if Application doesn't appear.
Double-click the password in the Keychain window, then click Access Control.
To allow access to this password by any application, select "Allow all applications to access this item."
To control access to this password, select "Confirm before allowing access."
To require the keychain password instead of the application password, select "Ask for Keychain password."
To add individual applications that can access this password, click Add (+) and navigate to the application you want to add.
IMPORTANT: If you select "Allow all applications to access this item" for an item in your keychain, you make it easy for a computer virus or other software to read the password.
Click Save.

Hi Matt OS X,
I suggest you go to, and then click on the 'Search this Forum' box at the right hand side top of this forum and type in 'Keychain Access'. A whole bunch of info about this problem will appear - including your own post. Browse around and you will find a 'fix' - I had the same problem and did just that! On the other hand there is a permanent 'fix' to be found - posted by 'Virtual1 Macmaster' over at 'MacFixit' in the Tiger Forum. You can download it to your desktop and then run it. I have used it and it really works!
The problem itself is no big deal, it's just annoying!
You can keep clicking on the 'Cancel' button in the Keychain Access window and it will go away - no big deal and no harm done. It's just downright irritating when it keeps popping up!

Keychain First Aid is built-in to modern versions of Keychain Access, so there's no need to download anything. Access it by opening Keychain Access in the Utilities folder (Applications/Utilities/), then selecting Keychain First Aid from the Window menu. Run it and you should no longer be prompted by Safari.

I'm still having a hard time to solve this problem. I did everything what you suggested me to do.
I went to Keychain app. and clicked on "Safari Forms AutoFill" and clicked on "Access Control" and selected "Allow all applications to access this item" and clicked on "Save"
then this window popped out:
I typed in password and it won't let me click on " Always allow". Only "Allow once" WhY?
i even got on Admin and changed the password and it still wont work? Wouldn't this help?

Big Mac is correct when he says that Keychain First Aid is built into modern versions of Jeychain access - but it did not work for me.
This might help - it's what I used to fix the same problem after trying Keychain First Aid to no avail.
As I said in my previous post, you will have to go to the MacFixit 'Tiger' forum to download Virtual1's "fix" if you need it. Here below is a copy of Virtual1's post - I hope it helps!

You probably force changed your password using the boot CD. At login, the system tries to use your login password to unlock your keychain. If you force changed your login pw, the keychain password remains unchanged and cannot be unlocked with the new password at login, so it remains locked. When an app tries to access the keychain, it then prompts you to unlock it. If you don't remember your previous password, you cannot unlock it and the information in the keychain can never be recovered.
Go into (home)/Library/Keychains/ and delete everything in that folder. (may be up to 3 items in there) Logout. Login.
Run safari and go to google.com, this will try to access forms autofill in the keychain. It will ask if you want to reset it to defaults. Alllow this. It will then ask for a password, use your login password. It may ask a third time if you really want to reset it, allow it.
Now run mail. Get new mail. It will prompt for your email password. Type it in and check the box to remember the password.
That should have you all fixed up.
I also made a "fix" for this for less experienced computer users where I work. You can download the applescript here .
