duplicate names in Finder's Recent Places and PS filters

Running 10.3.9 and in the Finder or certain applications like Photoshop, certain elements like recently opened files in Finder or even the filters in the Filter menu in Photoshop are being duplicated. See the below screen shots.

What could be causing this strange effect? I ran TechTool just to make sure and it didn't find or fix anything that would appear to be related.
The machine does have 2 internal and 1 external HD. The 2nd internal HD does not have much data on it but the external is used to back up (using Synk) the primary drive's data.
I saw on another thread that this effect could happen if their is an exact bootable image on 1 of the drives but to the best of my knowledge (this is a co-worker's machine) there is not.

Are you using SuperDuper? The reason I ask, is that I too noticed a "duplicate" issue (but in Finder's Open With...) after using SuperDuper. So I asked Dave Nanian, the developer, if he had insight. His reply:
"The reason you see multiple items, after using SuperDuper!, is because you're creating new copies of the applications (on the backup). Since the paths to these applications are different, LaunchServices puts them in again."
Rebuilding the Launch Services Database with Cocktail, OnyX, (or similar utilities) is a solution.
Maybe this is also applicable to your situation...

No never used that program. I guess I could try using Cocktail or OnyX to rebuild the Launch Services DB, as you suggested, to see if that works?

It can't hurt. And it just may work. Sorry, I just don't know, but would appreciate finding out should you give it a go.

Tried Cocktail to reset the launch services database. Did it 2x, once after a reboot. Same problem exists. Notice that in 1 of my screen shots, the effect also occurs in Photoshop using the Eye Candy 4000 filter, in addition to Recent Places.

Sorry my help wasn't.
Now this is really a stretch (guess)...
Have you recently installed any Photoshop updaters so there duplicate apps or preferences?
There are many clever chaps & chapettes around here. Maybe one of them will soon rise to the occasion.
