Best Formatting for OS X external disk?

I just got an external hard disk for backup purposes. It comes formatted as DOS, so I opened Disk Utilities, and there are options I don't remember seeing before:

Not only "Mac OS X Extended" and "Mac OS X Extended (Journaled)", but the option for each of those to be "(case-sensitive)" or ("case-sensitive, Journaled").

I don't have an OS X "Missing Manual" for Tiger, so can someone please enlighten me as to the significance and advisability of this "case-sensitive" option when it comes to formating a drive?

MacOS X Extended (Journaled) is the default these days.
I don't suffer from insanity, I enjoy every minute of it.

Yes, but within either journaled or UNjournaled options, there is the further option to choose (or not) "case-sensitive"...

I'm wanting to know what part of system operation will be affected by choosing "case-sensitive" -- something I never saw mentioned before (let alone given as an option).

Since the choices are Mac OS Extended (Journaled), Mac OS Extended, Mac OS Extended (Journaled, case-sensitive), etc. I thought saying Mac OS Extended (Journaled) was sufficient. You don't need case-sensitivity for now.

Case-sensitive is not supported for volumes on which you install OS X. It may work, but it's not supported.

You should *always* use journaled, unless you want the drive to take 30-60 minutes to come up successfully if you do an unclean shutdown -- as the drive must run fsck to rebuild some TOC information in this case.

Thanks for the clarification...

Probably I'd never seen the option before because I'd never been initialzing an external (and OS-free) drive before. CatOne's explanation helps me understand the "case-sensitive" issue.

I also appreciate understanding why I need to employee journaling, even on backup drives.
