1) DL Onyx, and use it's "rebuild LaunchServicesDatabase" function

2) Open terminal app and paste in this:

/System/Library/Frameworks/ApplicationServices.framework/\
Frameworks/LaunchServices.framework/Support/lsregister \
-kill -r -domain local -domain system -domain user

push enter and put in your admin pw and whahlah, all better now
