I just noticed that i have two times sleep, restart and shutdown??
How can I get rid of this double vision
i already restarted and nothing happened''

Based on your other thread, it looks like you've been playing around and installed something or enabled a hidden default to enable the extra menu items. They aren't duplicates. The commands with "..." at the end will ask for confirmation when chosen. The commands without a "..." at the end will not ask for confirmation. You also have two Force Quit items. One brings up the Force Quit window and the other force quits the current active application. Did you install or do something to enable that? If you did, that is your likely culprit.

i have not installed anything for over 1 month and this has appeared only yesterday
with the other thread the voice thing randomly turned on even though my speakers were working fine i noticed the box and not the actually voice as my speakers were turned off

I just did a quick test and it was VoiceOver that was the culprit.

Me too.
For some reason, Voiceover activated itself out of the blue and the repeating menu items appeared momentarily.
Only recent change in the system was update to newest Safari 3, but that was a while back.

odd... all of these seemed to happen after upgrading the software and also my quicktime player refuses to play .avi only freezes (not responding)

No, this happened when you turned on Voice Over. The QuickTime issue is unrelated. Make sure you have the latest version of Perian if you're using that. The update check is in System Preferences.
