How often should Safar be asking for permission to use my keychain?

It should ask once if you click "Always allow".

Hmm. I don't seem to be getting an "always allow" option. Perhaps there's a window I need to expand, like the new Print window.

Did you modify Safari? e.g. with a third party utility such as PithHelmet, etc?

Or did you muck around in the resources on disk?

This is a code signing issue. Something is invalidating the signature. If so, it's working as intended. Apple disables the "always allow" option in that case.

The signature can be invalidated in one of two ways: modifying the application on disk, or modifying it in memory.

I didn't modify Safari, but I did manage to mess up my keychain. Been trying to repair the damage ever since.

Are you using any APE haxies? InputManagers? SIMBL extensions (another InputManager thing)?

If not, then the problem might really be your keychain. Have you tried creating another user?

Not using any input managers, haxies or SIMBL extensions of any sort. I think it is my keychain, but I wanted to check with youse guys about Safari's behavior first.
