The plugin is most likely in either /Library/Application Support/SIMBL/Plugins or <home>/Library/Application Support/SIMBL/Plugins. You might also look (and trash everything) in /Library/Input Managers and <home>/Library/Input Managers.
You could have found the plugin yourself using Finders search (File->Search) then putting its name into the search criteria box, then selecting "Other..." from the search criteria popup and searching for and enabling the "System files" criterium. Then setting that to "included". By default Spotlight does not include system files in its search results.

There's another fix; simply edit the file:
/Library/Application Support/SIMBL/Plugins/Avatars.bundle/Contents/Info.plist
Find the number after MaxBundleVersion and change it to something much larger. This tells Address Book that the plugin will work with later versions. As far as I can tell, the plugin still works just fine.
