New windows open in back, not foremost

When I open a new application, it does not appear foremost, but behind every other running application. Apple-tab shows the new application on the far right as if it had just been hidden. I have to select the app again to bring it forward.
I saw this error occasionally on a long-time installation (12 months), and I'm noticing it now on my brand new 15" week old installation.
Restarting the finder does not resolve the issue. Anyone else seen this problem?

This happens to me all the time at work on Windows XP Pro. Like in Outlook when I try to make a new mail message, it opens behind the major Outlook window. Sorry. I'm not much help.

Create a new user, and check whether it happens to that user.
(not a fix, but perhaps a helpful diagnostic?)

I have had this happen a few times and a reboot is the only thing that fixes it.

sometimes... I'll simply log out and log in... or restart the finder.

The log out and log in is interesting, I'll try that next. The problem is intermittent, and a recent logitech driver installation forced a reboot and cleared the problem. Restarting the finder wasn't fixing it.
I wonder if we could trace it to an application. Yeserday it seemed to start happening (this is an extreme guess) after launching BBedit for the first time on the new laptop and selecting apple-~ for switching between windows when prompted by the new version. I've also mentally associated it with X11.

I don't have X11 on any of my Macs, and have seen the problem on all of them.

I get this with mail.
If I use services to send a link, the new mail window appears behind the main mail window.
But then, since I keep forgetting services is there, it doesn't happen that often. It's just annoying when it does.

Yes, I get this from time to time also.
10.3.5, G4/800.
Have not noticed any relationship btw what I'm doing and when the funny behavior starts.
