Custom Dictionaries in Word X et al.

It never ceases to amaze me how Word manages to screw you around so much. I don't know about the rest of you, but I've had it up to HERE with the "Custom Dictionary" function (and that's just for starters...). In my job I have to write a lot of technical reports, and rather than seeing a sea of wavy lines, I set up a custom dictionary to which I can add new words as they come along. Great. However, after a couple of weeks, and for no apparent reason, the "Add" feature becomes greyed out from the contextual menu. My heart sinks and I wander off to have a strong coffee (and a bottle of whiskey). A living hell ensues, in which I try renaming the dictionary, copying and pasting its contents, or whatever I can to try and get Word to recognise it and to use it again. Usually I'm unsuccessful, and have to start all over again. Same goes for the language version option. Because I live in Australia, 's's are used in many cases where a 'z' would be used in America. Same goes for English (UK). Changing over to English (AUS) or English (UK) almost *never* corrects this problem ('problem' for people outside the US), which has recurred since I first used Word years ago. Why have these options if they never work?
OK, to the question, and the point of this exercise:
Does anyone, *anyone* (!) have a fool-proof means of setting up a custom dictionary that *(heaven forbid!)* would be useable for the rest of the conceivable life of Word X on my machine. [But maybe then I wouldn't upgrade to the next version in the hope that the problem was *finally* fixed...]
