I have this in my Login Items, but I can't remember how it got there. Does anyone know what app this contextual menu plugin is associated with?

Did you do Get Info on the plug-in? That should tell you which company it belongs to which should be a strong hint to which app it belongs.

Contextual Menu plug-ins don't belong into Login Items (whatever that is). Remove it from there.
