I'd call it "Use it if it still works but maybe don't leave it plugged in overnight" bad. Also you should apply tape.

Believe it or not, just wear and tear, though I blame Apple because that cord turned yellow at the MagSafe after a year. I suppose the real wear was that the magnet was so weak it kept getting yanked off every time I moved the laptop.

I plan on using electrical tape to seal it up, but I want to make sure I'm not living with a fire hazard I should replace.
Also I'll be angry if I have to replace it because the cord was junk.

Pretty bad, I'd definitely trim more often and occasionally use a file to clean up the edge of the nail.

If no metal is exposed in the wire then electrical tape is fine. But if there is I wouldn't risk it.

I never liked those square MagSafe connectors. For whatever reason, the smaller, side-connecting ones seemed far more secure.

I agree the design is poor, but the rubber near the end went yellow fairly soon, I think from the heat. Nowhere else on the cord did the plastic yellow.

Seriously. Pitch it.
That model has a firestarter rep. Poor connection (my old one connects tightly), cooked jacket (a symptom of a poor connection), and significant fraying. It's not worth risking your notebook or house. It won't get better if you give it more time.

Not sure how that would happen. Certainly defective and needs to be replaced, not repaired. My MacBook Pro from 2012 looks brand new still (just dirty).

It is not uncommon. Mine (2011MBA) has something similar, although less severe. I suspect that dodgy cables that break are part of the reason that the charger cables to the new MBPs are much thicker and stiffer, and the charger no longer comes with the hooks for rolling up the cable.

The new Mac Pro has up to 30 MB of cache inside the processor itself. That's more than the HD in my first Mac. Somehow I'm still running out of space.
