I ran "fsck -y" in single mode. The result was:

fsck -y
/dev/rdisck0s2
root file system
checking HFSPLUS volume
checking extents overflow file
checking catalog file
disk0s2: 0x8 (undefined)
invalid key length
(4, 13069)
sh-2.05a#


What this mean??

Not sure, but it could be a corrupt directory, or possibly a bad volume table of contents on the drive. General rule of thumb with fsck is to keep running it until either: 1) it reports no errors, or 2) its error message hasn't changed since the previous run.

If it's a directory problem, DiskWarrior should be able to help (as it will build a totally new directory). DiskWarrior is worth having around in any event.
