Fairly often, when checking my log-in items, I find that most that were on my list there have disappeared. What's wrong and how can I fix it?

I too have had that problem, Ergo. You are not alone. Alas, I know not the solution.

I also posted this issue at another forum and it was suggested that I delete the com.apple.loginitems.plist. It might have gotten corrupted I then found that for some inexplicable reason, my Users/Preferences folder was missing....I cannot account for that, dunno how that happened. So I reinstalled my User/Preferences folder using 'Pacifist' and will now wait and see if my log-in items stay put now.
Possibly, the lack of the .plist was the reason for the disappearing log-in items. Stay tuned!
