Can you hold down the "C" key to boot from the CDROM/DVD with the bluetooth keyboard?

In other words, does the bluetooth get recognized at the hardware level (unlikely) prior to the OS loading or only when the OS is loaded?

Yes, when using either Apple's internal Bluetooth module, or with a D-Link DBT-120 with the latest Apple firmware, then the startup key combinations will work. However, as soon as the startup process has begun, control of Bluetooth is transferred to the OS, so while you can use the Bluetooth keyboard to boot from CD, once the CD has actually booted, it will not work, since the necessary preferences (that is, the ones storing the "pairing" between your Mac and the keyboard) are not present.
