Bad keystroke leads to 251 million stock buy
A Taiwan stock trader mistakenly bought $251 million worth of shares with a misstroke of her computer keyboard, meaning her company is looking at a paper loss of more than $12 million and she is looking for a new job.
You know, a simple "are you sure?" dialog box would have saved this person's job…

If that company were smart, they'd say this: "Fire you? Why would I do that? I just spent $12 million dollars training you!"
(That's from a management class I took, where a boss said just that to someone who'd goofed up, though it was only to the tune of $10,000 IIRC.)
But it's true: unless the person is a blithering idiot, they won't make that mistake, or any like it, ever again.
