...like Apple has on a lot of their apps where the image is reflected at the bottom.

I have Photoshop but I'm nowhere an expert.

My quick technique would be to duplicate the layer, flip vertical, create a layer mask and apply a gradient white to black in the area I'd like to fade.

I created a copy of the original image, flipped it vertically and pasted that into another layer in the original.

mask and gradient not done yet.

I need to modify the flipped layer so that the perspective is such that the layer is nearly flat in relation to the original.

How do I do that?

You can easily do it on the web yourself:

Reflection.js

Hit Cmd-T on the flipped layer to go into Free Transform mode. From there, you can squish the image vertically and even skew the bottom corners a bit wider to add a perspective effect. Let me know if that works.

That Reflection.js is great at doing image reflections and a bit quicker than scriptaculous and jQuery plugins as well.

If you're on a webkit browser you can even use CSS3-style reflections explained here.

alternatives to those mentioned:
* MS word 2008/2011 does a good job at creating reflections and file can be saved from word into jpegs.
* Alien Skin Impact filter / perspective shadow is really great for this.
* Picturesque | Acqualia this tool is quite nice too.
