Every once in a while, esp. when playing a video or doing something processor-intensive, the audio on my Cube 450 will hiccup, cut out, or get distorted. An entry to system.log is added that corresponds with each incident:

mach_kernel: USBF: 108587.620 AppleUSBOHCI[0x24d1000] Watchdog detected dead controller (hcca #: 48268, hc #: 48999)

Any ideas on how to fix it? Should I send a feedback to Apple?

Also, every so often, the sound dies altogether. If I unplug the USB apple pro speakers, loginwindow crashes:

I can't be of much help other than to say mine does the same thing. I won't even be doing anything other than having a song playing, and the sound will become muffled. Sometimes it'll also just stop. Used to happen all the time to me using 10.1, went away with 10.2.0, but came back after the updates started. If I unplug the speakers from the display, hello kernal panic. USB and audio just don't seem like a good combination to me.

I've been having this problem with my Cube for ages. There is no solution, and Apple doesn't seem to care much about Cube owners.

My work around for the windowmanager crash is to open the Sound system panel, switch the output to my iMic, unplug/plug in the usb, an switch it back to "Speakers".
